Most agreements need more than a name at the bottom. A date next to the signature, a checkbox for an acknowledgment, an amount filled into a fee line. If those are not fields, they get written in the margins or left blank.

A document can collect a date, a checkbox, or an amount, along with the signature. You place those fields where the contract asks for them. The signer fills them in the same pass as the signature.

A date field takes the date. A checkbox takes a yes. An amount takes the figure the line is asking for. They sit on the page with the signature, so the completed document has the answers and the signature together.

The signature is still the signature. The other fields are how the rest of the blanks get filled without a second form.
